The Tenth Way - Stumped
"A striker cannot be out stumped off a no-ball ..." - true
"... although it is perfectly legitimate for a quick thinking wicket-keeper to throw the ball to a fielder to effect a run-out!" - not true. The striker can't be run out from a no-ball unless he attempts a run, and in that case the WK can do the running-out without help.

The Eleventh Way
Poor young Abdul Aziz (only 18) was indeed hit in the chest, but not by a fast ball - it was a slow ball from a spin bowler - and it's theorised that the blow aggravated an undiagnosed heart condition. See for example his Wikipedia bio. (These days we hear all about Cardiac Risk in the Young, but in 1959 it was probably unknown.)
